Appointments may be made by calling 704.548.1221 weekdays. We offer early morning & late afternoon appointments for teachers & other working women. You will receive a reminder for your appointment by phone at least 2 days prior to your appointment time. We ask that you cancel or reschedule appointments at least 24 hrs in advance. Please arrive 15-20 minutes early to check-in and complete the required administatative paperwork. ALL PATIENTS ARE REQUIRD TO PRESENT THEIR INSURANCE CARD AND CO PAYMENTS AT CHECK-IN.

AFTER HOURS
Should you require medical care after hours, please call our office and leave your urgent message. Our phone system will automatically direct your emergency request to Dr. Petok. Please note, routine questions and prescription refills should be made during regular office hours.
